Editorial Many drugs that have been marketed for decades have become such a part of routine practice that they are taken for granted. Some are useful, with demonstrated efficacy and an acceptable adverse effect profile. Some others have no demonstrated clinical value beyond that of a placebo, and it is debatable whether they should remain on the market.

Dostarlimab (Jemperli°) in advanced or recurrent endometrial cancer with DNA repair defects

Marketing Authorisations Dostarlimab is an immunostimulatory anti-PD-1 antibody that has been authorised in the European Union for use in patients with advanced or recurrent mismatch repair deficient/microsatellite instability-high (dMMR/MSI-H) endometrial cancer, either in first-line treatment in combination with carboplatin and paclitaxel, or in second-line treatment as monotherapy.

Mavacamten (Camzyos°) in hypertrophic cardiomyopathy?

Marketing Authorisations Mavacamten, described as a selective and reversible inhibitor of cardiac myosin, has been authorised in the European Union in the treatment of symptomatic obstructive hypertrophic cardiomyopathy in adults. Does it extend survival in this situation, alleviate the symptoms of the disease (especially compared with a beta blocker or a calcium-channel blocker), reduce the incidence of serious complications, and avert the need for heart surgery? And what are its adverse effects?

Intravitreal VEGF inhibitors: systemic adverse effects

Adverse Effects A pharmacovigilance survey carried out by the French Health Products Agency (ANSM) identified 130 reports of cardiovascular adverse effects, mainly stroke, in patients treated with an intravitreal VEGF inhibitor. Two epidemiological studies, one conducted in the US and the other in Taiwan, quantified the risk of cardiovascular or kidney disease associated with intravitreal exposure to VEGF inhibitors.

Marketing Authorisations The adjuvanted RSVPreF3/AS01E vaccine is the first vaccine authorised in the European Union for the prevention of RSV-associated lower respiratory tract infections in adults aged 60 years or older. Valoctocogene roxaparvovec (Roctavian°) in haemophilia A?

Marketing Authorisations The gene therapy product valoctocogene roxaparvovec consists of a non-replicating adeno-associated virus serotype 5 (AAV5)-based vector that contains a DNA sequence encoding a variant of human factor VIII. It has been authorised in the European Union for the treatment of adults with severe haemophilia A on the basis of weak non-comparative data that suggest efficacy, but which declines over time.

Dupilumab in severe asthma in children: helpful and without risk of infections or cancer?

Queries and Comments Following the publication of Prescrire's analysis of evaluation data for dupilumab (Dupixent°) in severe asthma in children aged 6 years or older (Prescrire Int October 2023), members of Crisalis, an international clinical research network studying severe asthma, wrote to express their differing conclusions on dupilumab's benefits and harms in this situation.
