Psychotic episode: which oral neuroleptic to choose? A choice tailored to each patient, based on the adverse effect profiles of the options

Abstract

  • In addition to the fact that the efficacy of neuroleptics in treating the symptoms of psychotic episodes varies between patients, some of whom will already have tried these drugs, the main differences between the various neuroleptics lie in their adverse effects.

  • Amisulpride, paliperidone and risperidone cause fewer extrapyramidal effects than haloperidol, and do not appear to cause substantially more metabolic adverse effects.

  • Olanzapine, quetiapine and clozapine have less favourable harm-benefit balances than the other neuroleptics.
